Continuing Medical Education Funds Application
Applications may be submitted by e-mail as a .doc or .pdf attachment but must be accompanied by an original hard copy signed by the principal investigator and referenced co-investigators. Non-AMC Medical Staff applicants also must list a medical staff sponsor with accompanying signature. Applications are due no later than December 1, 2004.
A. Abstract: Summarize the proposed research project.
B. Background: State the rationale for the proposed research including any previous related research by the investigator(s), any relevant literature, and the anticipated contribution this project will make toward cancer prevention, diagnosis or treatment.
C. Objectives: Present the basic hypothesis of your research project. Provide related measurable objectives, primary and secondary as applicable, and anticipated outcomes. State a timeframe within which to meet these objectives.
D. Design: Provide an overview with, as applicable, information on:
- experimental design
- targeted population with inclusion and exclusion criteria
- recruitment strategies
- sample size determination and randomization procedures
- study duration and procedures; study endpoints
- potential risks to subjects; adverse events reporting mechanism
- data collection instruments and content
- proposed statistical analyses
- planned dissemination of results
E. IRB Approval: Projects including human subjects research need AMC - IRB approval before implementation -- an informed consent document will be required along with additional information per the separate IRB guidelines. Provide a copy of your preliminary informed consent.
Provide a line item description for each of the cost categories for which you are seeking funding with, as applicable, a per unit breakdown. Generally, direct payments compensating investigator time are not allowable. Budgets should not exceed $15,000. Provide a description of other support available, including in-kind contributions.

A. Overview: Summarize the proposed educational program.
B. Background: State the rationale for the specific program including any previous related activities by the applicant(s) and any relevant examples of similar successful projects. State the anticipated contribution this project will make towards cancer prevention, early diagnosis, clinical care or psychological support.
C. Objectives: Present an overall goal(s) for your project in terms of measurable educational objectives -- what do you expect the participants will be able to do, know, understand after the program that they couldn't before.
D. Project Design: Provide a copy of your completed or draft AMC application form(s) for CME credit, including the proposed brochure, financial disclosure forms, plans for evaluation, proposed speakers, program format and any other relevant documentation.
E. Coordination: Projects require coordination with, and the support of, AMC and Cancer Center leadership, as well as Graduate Medical Education before implementation. Provide documentation explaining and confirming this support.

Generally, direct payments compensating personnel time are not allowable although requests for honorariums for outside speakers are appropriate. Budgets should not exceed $10,000.
Provide a description of other support available, including in-kind contributions.
