Atlanta Medical Center offers a number of diabetes classes. A written physician referral is required. The referral should include your diagnosis and specify group or individual class. You can download a physician referral form to take to your physician by clicking here. (This form is in Microsoft Word format.)

NOTE: If you are attending a class at Atlanta Medical Center, please arrive at least 30 minutes beforehand to complete your registration. Please go to the Admissions department for the registration process.

All classes are in English unless otherwise noted.

Group Fast Track Basics and Gestational
Parts 1 & 2, Class are held on Thursdays  from 9:30 a.m. to 3:30 p.m. (except for the last Thursday of the month)

PLEASE NOTE:

Class participants should bring their signed physician referral form along with their A1c results and diagnosis, glucose testing equipment (if you already have this), a list of medications and one significant other (close friend, relative or caregiver).
